School Information and Team Rosters

• Player Eligibility Roster - required, used to submit the eligible players for a team (Player Eligibility Roster in Microsoft Word format)
• Team Data Sheet - required, used to submit information on the school, coaches, gym location, etc. for a team

Accidents, Emergencies, Injury Reporting

• Accident-Emergency Information - keep nearby with vital information that may be needed in case of an accident or emergency
• Athletes Emergency Information - keep nearby with emergency information for individual players
• Injury Reporting - used to report an injury to the Diocese

Parents Pledge

• Parent Participation Pledge - required, lists parents expected behavior

Physicals

• Physician-Parent Release - required
• PIAA Comprehensive Initial Pre-Participation Physical Evaluation
