School Information and Team Rosters

• Player Eligibility Roster - required, used to submit the eligible players for a team (Player Eligibility Roster in Microsoft Word format)
• Team Data Sheet - required, used to submit information on the school, coaches, gym location, etc. for a team

Referee Request Form

On occasion there are referees who you might not want officiating your home games. List the names of those referees on this form and the scheduler will try to accommodate your request.

Accidents, Emergencies, Injury Reporting

• General Accident-Emergency Information - Fill out this form and make it available to the appropriate individuals. Its a good idea to keep it in a Ziploc bag in your first aid kits - hoping you never need it.
• Individual Athletes Emergency Information - keep nearby with emergency information for individual athletes
• Policies Covering Participation in Athletics - used to report medical coverage information and injuries
